Choose Your Path

How will you be accessing services?

The first steps are a little different depending on how you’ll be accessing care. Find your starting point below, and we’ll guide you through what comes next.

Arizona DDD

Starting with a DDD referral

If you already have or are expecting a referral through Arizona DDD, your Support Coordinator is typically part of the process.

  1. Referral: Your Support Coordinator sends or assigns the referral.
  2. Intake: Our team contacts you to gather information and confirm next steps.
  3. Evaluation: When needed, a clinician completes an evaluation and develops recommendations.
  4. Approvals: Required plan-of-care and authorization steps are completed before ongoing treatment begins.
  5. Scheduling: Once approved, we coordinate recurring appointments based on service needs and clinician availability.
ESA

Using ESA funding

Families using Arizona Empowerment Scholarship Account funds can contact Simply directly to ask about current availability and next steps.

  1. Reach out: Tell us what type of therapy you’re looking for.
  2. Intake: We gather the information needed to determine whether Simply may be a fit.
  3. Payment setup: We confirm the applicable ESA payment process before an evaluation is scheduled.
  4. Evaluation: If clinically appropriate, an evaluation helps establish needs and goals.
  5. Scheduling: We coordinate recurring services based on clinician availability.
Private Pay

Starting as a private-pay family

Private-pay families can contact Simply directly. You do not need a DDD referral to begin the conversation.

  1. Reach out: Let us know what support you’re seeking.
  2. Intake: We review your needs, location, and current availability.
  3. Payment setup: We confirm rates and payment arrangements before an evaluation is scheduled.
  4. Evaluation: When appropriate, an evaluation identifies clinical needs and individualized goals.
  5. Scheduling: Ongoing appointments are coordinated based on the plan of care and clinician availability.

What Happens Next

Different starting points. One path forward.

Your exact steps depend on how you access care, but every pathway is designed to make sure the right referral, payment, evaluation, and approval requirements are completed before ongoing therapy begins.

1

We learn what you need

We review your goals, location, service needs, and how you plan to access services.

2

We confirm your pathway

Before evaluation, we complete the steps that apply to you, such as confirming a DDD referral or setting up ESA or private-pay arrangements.

3

A clinician evaluates

When needed, an evaluation helps establish clinical needs and individualized treatment goals.

4

We complete final requirements

For DDD services, this may include plan-of-care and authorization steps. We also confirm anything else needed before recurring treatment begins.

5

We schedule and begin

Recurring appointments are matched to clinician availability, service location, and the plan of care so therapy can get underway.

Good To Know

Timing can vary.

The amount of time between first contact and ongoing therapy depends on the referral, evaluation needs, required approvals, service location, and current clinician availability.

Our team will keep you informed about where you are in the process and what still needs to happen before services can begin.
