Effective: August 30, 2026

At a glance

You have rights regarding your health information, choices about certain uses and disclosures, and the right to ask questions or file a complaint. SimplyRN LLC is required to protect the privacy and security of your protected health information and follow the practices described in this notice.

Your Rights

When it comes to your health information, you have certain rights. This section explains those rights and some of our responsibilities to help you exercise them.

Get an electronic or paper copy of your medical record

  • You may ask to see or receive an electronic or paper copy of your medical record and other health information we maintain about you.
  • We will provide a copy or summary of your health information, usually within 30 days of your request. We may charge a reasonable, cost-based fee when permitted by law.

Ask us to correct your medical record

  • You may ask us to correct health information about you that you believe is incorrect or incomplete.
  • We may deny your request in some circumstances, but if we do, we will explain why in writing, generally within 60 days.

Request confidential communications

You may ask us to contact you in a specific way, such as at a particular telephone number, or to send mail to a different address. We will accommodate reasonable requests.

Ask us to limit what we use or share

  • You may ask us not to use or share certain health information for treatment, payment, or health care operations. We are not always required to agree, including when a restriction could affect your care.
  • If you pay for a health care service or item completely out of pocket, you may ask us not to share information about that service or item with your health plan for payment or health care operations. We will agree unless the law requires us to share the information.

Get a list of certain disclosures

You may ask for an accounting of certain disclosures of your health information made during the six years before your request, including who received the information and why. The accounting does not include every type of disclosure, such as many disclosures for treatment, payment, or health care operations. One accounting in a 12-month period is provided without charge; a reasonable, cost-based fee may apply to additional requests.

Get a copy of this notice

You may ask for a paper copy of this notice at any time, even if you agreed to receive it electronically. We will provide one promptly.

Choose someone to act for you

If a person has legal authority to act for you, such as a legal guardian or a person holding an appropriate medical power of attorney, that person may exercise your rights and make choices about your health information. We will verify the person’s authority before acting.

File a complaint if you believe your rights were violated

  • You may complain to SimplyRN LLC by contacting our Privacy Officer using the information below.
  • You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights at HHS.gov, by calling 1-877-696-6775, or by writing to 200 Independence Avenue, S.W., Washington, D.C. 20201.
  • We will not retaliate against you for filing a complaint.

Your Choices

For certain health information, you may tell us your preferences about what we share. If you have a clear preference in the situations below, tell us what you want us to do. We will follow your instructions when required by law and otherwise as reasonably possible.

  • Share information with family members, friends, caregivers, or others involved in your care or payment for your care.
  • Share information in a disaster-relief situation.

For marketing purposes and the sale of your health information, we generally must obtain your written authorization. SimplyRN LLC does not sell protected health information.

Our Uses and Disclosures

We typically use or share your health information in the following ways.

We may use and share health information to operate our practice, improve the quality of care, supervise clinicians, manage scheduling and services, conduct quality and compliance activities, and contact you when necessary.

Bill for your services

We may use and share health information to bill and obtain payment from health plans, Arizona DDD, or other entities responsible for payment. We may also use information as needed to administer ESA or private-pay arrangements when applicable.

Other uses and disclosures permitted or required by law

We may use or share your information in other circumstances permitted or required by law, including to:

  • Help with public health and safety activities, such as reporting certain diseases, suspected abuse or neglect, or preventing or reducing a serious threat to health or safety.
  • Comply with federal, state, and other applicable laws and with health oversight activities.
  • Conduct research when applicable legal requirements are satisfied.

Substance Use Disorder Records

To the extent SimplyRN LLC creates or maintains substance use disorder patient records that are protected by 42 CFR Part 2, additional federal confidentiality protections apply. We will not use or disclose those Part 2 records for civil, criminal, administrative, or legislative proceedings against you unless you provide written consent or the disclosure is authorized by a court order and subpoena as required by law.

Our Responsibilities

  • We are required by law to maintain the privacy and security of your protected health information.
  • We will notify you promptly if a breach occurs that may have compromised the privacy or security of your information, as required by law.
  • We must follow the duties and privacy practices described in this notice and make a copy of this notice available to you.
  • We will not use or share your information other than as described in this notice unless you authorize us to do so in writing. You may revoke an authorization in writing at any time, subject to actions already taken in reliance on it and other limitations permitted by law.
  • When Arizona or another applicable law provides greater privacy protection than HIPAA, we will follow the more protective law.
